Intraoral examination revealed intra-alveolar intrusion of maxillary tooth no. 12 more than 7 mm, avulsion of the maxillary tooth no. 11 from the socket, vestibular and horizontal displacement of the tooth No. 21, associated with alveolar fracture. This trauma had been treated by two endodontists, a prosthodontist, a surgeon and an orthodontist.
